These forms are downloadable or printable and may be returned via email, put into the box outside of the GRYM office at Garden Ridge or given to Trane directly.

INFO AND RELEASE FORM

Must be filled out and turned in for every STUDENT PARTICIPANT before going on any trips with the youth group

FINANCIAL ASSISTANCE REQUEST FORM

Must be filled out and turned in for any STUDENT requesting financial assistance for a GRYM event on or before the payment/sign-up deadline for that event (Please note the policy on the form)

BACKGROUND CHECK CONSENT

Must be filled out and turned in for every ADULT VOLUNTEER before being involved in the youth ministry in any way (to ensure a safe environment for the teens)

DENVER PACKET: Info Packet/Packing List

Important info about the Denver Mission Trip, including a packing list.

DENVER FORM: Volunteers of America Parental Consent (under 18)

Must be filled out by ALL DENVER MISSIONARIES 17 YEARS OLD OR YOUNGER before the trip.

DENVER FORM: Volunteers of America Waiver (all)

Must be filled out by ALL DENVER MISSIONARIES (all ages) before the trip.

DENVER FORM: City Connexx Registration and Waiver (all)

Must be filled out by ALL DENVER MISSIONARIES (all ages) before the trip. This is an online form.

DENVER FORM: Independent Whitewater Waiver (all)

Must be filled out by ALL DENVER MISSIONARIES (all ages) who wish to whitewater raft before the trip.

TREK PACKET AND FORMS

All teen and adult High School Trek participants must fill out and return the GRYM and TEAM Ascend forms in this packet BY SUNDAY, MAY 21. This packet also includes vital info about what to bring and how to prepare for the trip. Please read it carefully.

GROUP DYNAMIX RELEASE FORM: Summer Blowout Lock-In

All 6th-12th graders attending the Summer Blowout Lock-In at Group Dynamix (August 14) must fill out and return this form to participate.

CAMP ENCOUNTER REGISTRATION

CAMP ICHTHUS: STUDENT APPLICATION

Students who have COMPLETED 6th-11th grade, fill out the camper application and return it to Trane Wyatt (see calendar for deadline)

CAMP ICHTHUS: COUNSELOR APPLICATION

Those who have/will have completed 12th grade by this summer and who want to volunteer as a camp counselor, fill out the counselor application and return it to Trane Wyatt (see calendar for deadline)

BABYSITTING FORM

Leaving your child/children with GRYM for babysitting? Save time by printing and filling out this form so it's ready to turn in at drop-off.

SENIOR BIO

All current grads-to-be, please fill out this brief questionnaire before the Senior Sunday deadline and email it to Trane.
